    Summary

    Patients with severe carotid artery disease and high intraocular pressure did not develop glaucoma. This suggests low blood pressure is not the sole factor in glaucoma progression, highlighting unknown susceptibility factors.

    Background:

    • Glaucoma is often associated with low ocular blood flow.
    • Severe carotid artery disease can compromise blood flow to the eye.

    Observation:

    • Five patients with severe bilateral carotid artery disease and elevated intraocular pressure were monitored for 3 to 12 years.
    • These patients did not exhibit glaucomatous optic disc or visual field changes during the follow-up period.

    Findings:

    • Contrary to common assumptions, low systemic or ophthalmic artery blood pressure was not a determinant for rapid glaucoma progression in this cohort.
    • The absence of glaucomatous changes despite risk factors indicates that intraocular pressure and carotid disease alone are insufficient predictors.

    Implications:

    • This study challenges the established link between reduced ocular perfusion and glaucoma progression in patients with significant carotid artery disease.
    • It highlights the critical role of other, yet unidentified, susceptibility factors in the pathogenesis of glaucoma, even with elevated intraocular pressure.
