Pacemaker update 1984. Part II. The pacemaker syndrome.

M M Parker, L Lemberg

Heart & Lung : the Journal of Critical Care
    |July 1, 1984
    PubMed
    Summary

Ventricular pacemakers can cause retrograde conduction, leading to heart issues and symptoms like syncope. Switching to a dual-chamber pacemaker can restore AV synchrony and prevent these complications.

    Area of Science:

    • Cardiology
    • Biomedical Engineering
    • Electrophysiology

    Background:

    • Patients with normal AV conduction receiving ventricular pacemakers are susceptible to retrograde conduction.
    • Retrograde conduction activates the atria after ventricular activation, disrupting normal cardiac function.

    Observation:

    • This retrograde activation causes blood to flow backward from ventricles to atria.
    • Jugular and pulmonary veins become flooded due to this abnormal retrograde flow.

    Findings:

    • The retrograde flow can lead to reduced cardiac output and congestive heart failure.
    • Symptoms such as weakness and syncope may occur as a consequence.

    Implications:

    • A change to a dual-chamber pacemaker is often necessary to correct this issue.
    • Dual-chamber pacemakers can restore atrioventricular (AV) synchrony, mitigating retrograde conduction problems.
    • This intervention improves patient outcomes by preventing heart failure symptoms.
