    Styloid pain syndrome is a newly recognized complication of mandibular osteotomy, distinct from temporomandibular joint pain. This condition, caused by stylomandibular ligament stretching, may necessitate further surgery for symptom relief.

    Background:

    • Temporomandibular joint (TMJ) pain is a known complication of mandibular surgery.
    • Styloid pain syndrome (SPS) is typically associated with tonsillectomy or occurs de novo.
    • Existing literature on SPS focuses on the styloid process, stylohyoid attachments, or carotid artery involvement.

    Observation:

    • This case report details a patient experiencing stylohyoid pain syndrome following mandibular osteotomy.
    • The pain was attributed to the stretching of the stylomandibular ligament, a previously unrecognized cause.
    • Conservative management was insufficient, requiring surgical intervention.

    Findings:

    • Mandibular osteotomy can lead to stylohyoid pain syndrome via stylomandibular ligament stretching.
    • This represents a novel complication of mandibular osteotomy.
    • Surgical intervention was effective in resolving the patient's symptoms.

    Implications:

    • Clinicians should consider SPS in patients presenting with neck or throat pain post-mandibular surgery.
    • Recognition of this complication may alter surgical planning and post-operative care protocols.
    • Further research is warranted to elucidate the pathophysiology and incidence of SPS after mandibular osteotomy.
