On the genesis of "dry socket".

D W Nitzan

Journal of Oral and Maxillofacial Surgery : Official Journal of the American Association of Oral and Maxillofacial Surgeons
    |November 1, 1983

    Summary

Dry socket (alveolar osteitis) may result from blood clot breakdown, not failure to form. Research suggests bacteria, particularly Treponema denticola, likely cause this fibrinolysis, leading to severe pain after tooth extraction.

    Area of Science:

    • Oral surgery
    • Dental pathology
    • Microbiology

    Background:

    • Dry socket (alveolar osteitis) is a common complication following tooth extraction.
    • Existing theories on dry socket pathogenesis include failed blood clot formation or clot lysis.
    • Clinical presentation often contradicts the failed clot formation theory.

    Purpose of the Study:

    • To review existing literature on dry socket pathogenesis.
    • To investigate the role of fibrinolysis in dry socket development.
    • To identify potential bacterial involvement in the lysis of the blood clot.

    Main Methods:

    • Literature review and analysis of existing data on dry socket.
    • Examination of theories regarding blood clot formation and lysis.
    • Postulation of bacterial involvement based on accumulated evidence.

    Main Results:

    • The theory of failed blood clot formation is challenged by clinical symptoms.
    • Fibrinolysis plays a role, but tissue activators alone do not fully explain dry socket.
    • Bacterial agents are implicated in the fibrinolytic process.

    Conclusions:

    • Treponema denticola may be a key bacterial agent in dry socket pathogenesis.
    • Bacterial-induced fibrinolysis is a more plausible explanation for dry socket.
    • Further research into specific bacterial roles is warranted.
