    Background:

    • The Human Leukocyte Antigen (HLA) system plays a crucial role in immune response.
    • Genetic factors contributing to type 2 diabetes (T2D) susceptibility are not fully understood.
    • Previous studies suggest a potential link between HLA antigens and diabetes.

    Purpose of the Study:

    • To investigate the association between specific HLA-A, B, and C antigens and type 2 diabetes.
    • To compare HLA antigen frequencies in T2D patients with a healthy Finnish population.
    • To explore potential heterogeneity in T2D based on HLA antigen profiles.

    Main Methods:

    • Determined HLA-A, B, and C antigens in 227 T2D patients (age 35-70).
    • Compared patient HLA antigen frequencies with those of Finnish blood donors.
    • Categorized patients into non-insulin dependent (diet/oral medication) and insulin-dependent groups.

    Main Results:

    • HLA-Cw4 antigen significantly increased in non-insulin dependent T2D patients (RR 2.67, p=0.00005).
    • HLA-Bw35 antigen also increased in non-insulin dependent T2D (RR 1.6, p=0.007), potentially due to linkage disequilibrium with HLA-Cw4.
    • Insulin-dependent T2D showed decreased HLA-B7 and increased HLA-B8, B12, and B15 frequencies compared to controls.

    Conclusions:

    • Genetic factors within the HLA region may influence susceptibility to type 2 diabetes.
    • Maturity onset (type 2) diabetes appears heterogeneous regarding HLA antigen associations.
    • Specific HLA antigen profiles may correlate with different T2D subtypes or progression.
