Chemotherapy can be safely administered to patients with hepatic porphyrias, including porphyria variegata. Treatment decisions for these patients should prioritize managing their underlying cancer.

    Area of Science:

    • Hepatology
    • Oncology
    • Genetics

    Background:

    • Hepatic porphyrias are rare genetic disorders affecting heme biosynthesis.
    • Patients with hepatic porphyrias may develop disseminated malignant diseases requiring chemotherapy.
    • The safety of chemotherapy in this patient population is a significant clinical concern.

    Observation:

    • This study reviewed the clinical courses of 5 patients with hepatic porphyrias (4 with porphyria variegata) undergoing chemotherapy.
    • Antineoplastic agents were administered to these patients.
    • No porphyric crises were precipitated by the chemotherapeutic agents used.

    Findings:

    • Chemotherapy can be safely used in patients with hepatic porphyrias.
    • The use of antineoplastic drugs did not trigger porphyric crises in the observed cases.

    Related Experiment Videos

  • The primary treatment focus should remain on the management of the malignant disease.
  • Implications:

    • Oncologists can consider chemotherapy for patients with hepatic porphyrias without undue fear of precipitating porphyric crises.
    • This finding supports evidence-based treatment protocols for cancer in patients with rare genetic disorders.
    • Further research may explore specific chemotherapy regimens and their long-term effects in this population.
