    Summary

    Pregnancy hydronephrosis, or kidney swelling, is common and usually mild. Severe cases are rare, typically starting in the second trimester, and rarely worsen significantly.

    Background:

    • Physiological hydronephrosis is a common finding during pregnancy.
    • Its progression and underlying mechanisms require further elucidation.

    Purpose of the Study:

    • To longitudinally assess the progression of physiological hydronephrosis during pregnancy using ultrasonic measurements.
    • To evaluate the factors influencing the severity and onset of hydronephrosis.

    Main Methods:

    • A longitudinal study involving 56 asymptomatic pregnant patients.
    • Ultrasonic measurement of calyceal diameters throughout the three trimesters of pregnancy.
    • A grading system based on maximum calyceal diameter was employed.

    Main Results:

    • 89% of patients exhibited at least mild hydronephrosis before delivery, with right-sided prevalence.
    • Severe dilatation occurred in 10.7% of cases, predominantly beginning in the second trimester.
    • Progression from mild to moderate/severe dilatation in the second trimester was uncommon, as was progression from moderate to severe in the third trimester.

    Conclusions:

    • Findings support the mechanical theory of pregnancy-induced hydronephrosis.
    • Severe hydronephrosis is infrequent and typically originates in the second trimester.
    • Postpartum regression of calyceal dilatation is not rapid in the immediate puerperium.
