    Summary

Patient problems better predict treatment plans and long-term outcomes than diagnoses alone. Combining problems and diagnoses offers the most accurate prediction for psychiatric patient treatment and results.

    Area of Science:

    • Psychiatry
    • Clinical Psychology
    • Health Informatics

    Background:

    • Current diagnostic systems like DSM-II are foundational in psychiatric care.
    • The utility of problem-focused nomenclature in predicting treatment and outcomes requires further investigation.
    • Integrating patient-reported problems with formal diagnoses may enhance clinical utility.

    Purpose of the Study:

    • To evaluate the predictive value of a problem-focused nomenclature compared to DSM-II diagnoses.
    • To assess the efficacy of problems and diagnoses in predicting psychiatric treatment plans and patient outcomes.
    • To determine the optimal combination of problems and diagnoses for predicting treatment and outcomes.

    Main Methods:

    • Coded patient problems, diagnoses, and treatment plans from 779 psychiatric inpatient records.
    • Compared the predictive power of problems versus DSM-II diagnoses for treatment plans.
    • Assessed problems and diagnoses as predictors of patient outcomes at two months and one year.

    Main Results:

    • Patient problems were superior to DSM-II diagnoses in predicting treatment plans.
    • The combination of problems and diagnoses best predicted psychotropic medication treatment.
    • At two months, problems and diagnoses equally predicted outcomes; their combination was best.
    • At one year, problems explained significantly more outcome variance than diagnoses.

    Conclusions:

    • Problem-focused nomenclature offers valuable predictive insights for psychiatric treatment and outcomes.
    • Integrating patient problems with diagnostic criteria enhances prediction accuracy.
    • Problem-focused approaches may be more robust than diagnoses alone for long-term outcome prediction in psychiatry.
