    Summary

    Postoperative endophthalmitis from Staphylococcus epidermidis is rare after scleral buckling. Prompt diagnosis and aggressive treatment are crucial for preserving visual acuity in these serious infections.

    Background:

    • Scleral buckling is a common surgical procedure for retinal detachment.
    • Postoperative endophthalmitis is a rare but severe complication following intraocular surgery.

    Observation:

    • Two cases of endophthalmitis caused by Staphylococcus epidermidis after 10,000 scleral buckling operations are presented.
    • Bacterial entry is suspected through scleral perforations used for subretinal fluid release and air injection.

    Findings:

    • Case 1: Despite extensive treatment (vitrectomy, corticosteroids, antibiotics), visual acuity declined to light perception.
    • Case 2: Intensive therapy preserved visual acuity at 6/12 (20/40).

    Implications:

    • Early diagnosis and rapid, aggressive intervention are critical for favorable visual outcomes in postoperative endophthalmitis.
    • Minimizing bacterial contamination during scleral buckling procedures is essential.
