    Strongyloidiasis, a soil-transmitted nematode infection, is often overlooked in patients with major illnesses. Early diagnosis is crucial, as it can mimic opportunistic infections and be fatal if missed.

    Background:

    • Strongyloidiasis is a nematode infection prevalent in the southeastern US.
    • Diagnosis often occurs in patients with significant comorbidities.

    Observation:

    • A study at a Veterans Affairs Medical Center diagnosed 33 cases of strongyloidiasis.
    • Most patients presented with concurrent major illnesses like lung disease, infections, or cancer.
    • Gastrointestinal symptoms were less common, and skin rash was infrequent.

    Findings:

    • Eosinophilia, elevated IgE, and skin anergy were frequently observed.
    • Atypical presentations included proctitis, colitis, and inflammatory bowel disease exacerbation.
    • One case of hyperinfection syndrome was only diagnosed post-mortem.

    Implications:

    • Strongyloidiasis can present as an opportunistic infection.
    • Physicians, even outside endemic areas, should consider strongyloidiasis in at-risk patients.
    • Increased awareness and diagnostic vigilance are necessary for timely treatment.
