    Summary

    Optic nerve drusen may stem from abnormal congenital blood vessels in the optic disc. These vascular abnormalities allow plasma leakage, forming extracellular deposits that cause optic nerve drusen.

    Background:

    • Optic nerve drusen are calcified deposits within the optic nerve head.
    • Their exact etiology remains unclear, with theories ranging from developmental anomalies to degenerative processes.

    Observation:

    • Histologic examination of a patient with pseudopapilledema revealed numerous perivascular optic nerve drusen.
    • Review of fluorescein retinal angiograms in patients with optic nerve drusen showed distinct vascular abnormalities compared to controls.

    Findings:

    • Abnormal branching patterns of optic disc vasculature were observed.
    • Unusually large blood vessels connecting superficial and deep optic disc circulations were present.
    • Increased capillary networks within the optic disc were noted.

    Implications:

    • These findings suggest that optic nerve drusen may result from congenital abnormalities in optic disc vasculature.
    • Vascular leakage of plasma proteins could serve as a nidus for extracellular material deposition, leading to drusen formation.
    • Understanding the vascular basis of optic nerve drusen may inform future diagnostic and therapeutic strategies.
