    Background:

    • Generalized lymphadenopathy was observed in 90 homosexual or bisexual men.
    • Patients had histories of multiple sexually transmitted diseases and drug use.
    • Lymph node histology revealed varied patterns including hyperplasia and involution.

    Purpose of the Study:

    • To investigate the clinical, pathological, immunological, and genetic aspects of generalized lymphadenopathy in homosexual men.
    • To explore the association between lymphadenopathy and acquired immunodeficiency.
    • To identify potential risk factors and disease progression.

    Main Methods:

    • Epidemiologic, clinical, pathological, immunological, and genetic analyses were performed.
    • Lymph node biopsies were examined histologically.
    • Human Leukocyte Antigen (HLA) typing, specifically for HLA-DR5, was conducted.

    Main Results:

    • Three distinct lymph node histological patterns were identified.
    • A significant increase in the frequency of HLA-DR5 was observed in patients compared to controls.
    • All patients exhibited impaired cell-mediated immunity.

    Conclusions:

    • Generalized lymphadenopathy is suggested to be part of a spectrum of acquired immunodeficiency.
    • This spectrum includes opportunistic infections, Kaposi's sarcoma, and malignant lymphomas.
    • Severe immune dysfunction preceded the development of these opportunistic conditions in 15 patients.
