[Exercise tests before and after aortocoronary bypass].

    Summary

Coronary artery bypass grafting significantly reduces chest pain during stress tests in patients with coronary artery disease. Complete revascularization through bypass surgery improves exercise tolerance and delays the onset of ischemic pain.

    Area of Science:

    • Cardiology
    • Cardiovascular Surgery

    Context:

    • Coronary artery disease (CAD) is a leading cause of morbidity and mortality.
    • Stress testing is a common diagnostic tool for evaluating myocardial ischemia.
    • Coronary artery bypass grafting (CABG) is a surgical intervention for severe CAD.

    Purpose:

    • To evaluate the impact of coronary artery bypass grafting (CABG) on exercise-induced ischemia.
    • To assess changes in stress test parameters and patient-reported symptoms before and after CABG.

    Summary:

    • A study involving 83 patients undergoing CABG demonstrated a significant reduction in painful stress tests post-operatively (from 68 to 28 instances).
    • Improvements were most pronounced when all significant coronary artery lesions were addressed via bypass.
    • Maximal exercise performance did not significantly change, but the threshold for ischemic pain onset increased substantially.

    Impact:

    • CABG surgery leads to improved myocardial perfusion and delayed onset of angina pectoris.
    • Complete revascularization via CABG enhances functional capacity and quality of life in CAD patients.
    • These findings support CABG as an effective treatment for alleviating ischemic symptoms in coronary artery disease.
