    Summary

    Patients with cutaneous T cell lymphoma (CTCL) have a higher risk of developing a second malignancy, particularly white males. Prior chemotherapy and family history may increase this risk.

    Background:

    • Cutaneous T cell lymphoma (CTCL) is a group of rare cancers affecting the skin.
    • The incidence of secondary malignancies in CTCL patients requires further investigation.

    Purpose of the Study:

    • To determine the frequency of second primary malignancies in CTCL patients.
    • To compare cancer incidence in CTCL patients to the general population.
    • To identify potential risk factors for developing a second malignancy in CTCL patients.

    Main Methods:

    • Retrospective analysis of 15-year data from 63 CTCL patients at Duke University Medical Center.
    • Comparison of cancer incidence rates between CTCL patients and the general population.
    • Analysis of patient characteristics (prior chemotherapy, family history) to identify risk factors.

    Main Results:

    • 15.9% of CTCL patients developed a second malignancy, a higher frequency than previously reported.
    • Overall cancer incidence was 2.4 times higher in CTCL patients than expected.
    • White male CTCL patients had 3.3 times the expected cancer incidence.
    • Prior chemotherapy and a family history of malignancy were more common in CTCL patients with a second malignancy.

    Conclusions:

    • CTCL patients face an elevated risk of developing subsequent primary malignancies.
    • Specific patient factors, including prior chemotherapy and family history, may predispose CTCL patients to secondary cancers.
