Varicocele in youth.

R P Lyon, S Marshall, M P Scott

    The Western Journal of Medicine

    Summary

Varicoceles in boys, a common urologic condition, are often associated with smaller left testicles due to growth arrest. Surgical intervention is recommended for significant varicoceles or testicular size discrepancies.

    Area of Science:

    • Pediatric Urology
    • Andrology
    • Developmental Biology

    Background:

    • Varicoceles are a common cause of scrotal pathology in pediatric patients.
    • Testicular size discrepancy is a frequent concern in pediatric urology.
    • Understanding the etiology of testicular hypotrophy in adolescents is crucial for timely intervention.

    Purpose of the Study:

    • To investigate the association between palpable varicoceles and testicular size in adolescent males.
    • To determine if testicular hypotrophy in patients with varicocele is due to arrest of growth or atrophy.
    • To establish criteria for surgical correction of varicoceles in pediatric patients.

    Main Methods:

    • Retrospective chart review of 30 pediatric patients (8-18 years) with palpable varicoceles.
    • Clinical assessment of varicocele grade and testicular volume/size.
    • Correlation analysis between varicocele presence and left-sided testicular hypotrophy.

    Main Results:

    • Thirty boys aged 8-18 years with palpable varicoceles were identified.
    • A significant finding was left-sided testicular hypotrophy in 77% of these patients.
    • The smaller testis was present before puberty, suggesting growth arrest rather than atrophy.

    Conclusions:

    • Palpable varicoceles in boys are strongly associated with ipsilateral testicular growth arrest.
    • Early identification and management of varicoceles are essential to prevent long-term testicular hypotrophy.
    • Surgical correction is advised for pronounced varicoceles, significant testicular size discrepancy, or scrotal pain.
