    Summary

    Long-term amiodarone (AMD) therapy can cause corneal microdeposits in 98% of patients. However, these ocular effects are generally insignificant, do not threaten vision, and do not require routine ophthalmologic surveillance.

    Background:

    • Long-term amiodarone (AMD) therapy is associated with ocular complications.
    • Previous data on the incidence and prognostic significance of these effects were limited.

    Purpose of the Study:

    • To systematically evaluate the incidence and clinical significance of ocular complications in patients undergoing long-term amiodarone treatment.

    Main Methods:

    • Prospective follow-up of 175 patients on long-term amiodarone therapy.
    • Detailed ophthalmologic examination of 103 patients.
    • Assessment of corneal microdeposits and associated visual symptoms.

    Main Results:

    • Corneal microdeposits observed in 98% of examined patients, typically stable and dose-dependent.
    • Visual symptoms (photophobia, halos, blurred vision) occurred in 6% of patients, without visual acuity impairment.
    • Ocular abnormalities resolved within 7 months after drug withdrawal in 16 patients, with no permanent damage.

    Conclusions:

    • Ocular complications of amiodarone therapy are generally insignificant and do not pose a threat to vision.
    • Routine ophthalmologic surveillance may not be mandatory for most patients during long-term amiodarone treatment.
