    Background:

    • Recurrent herpes simplex keratitis (HSK) is a significant cause of vision loss.
    • Corneal superinfections complicate HSK management, necessitating identification of risk factors and causative agents.

    Observation:

    • A review of 15 culture-proven corneal superinfections in patients with recurrent HSK was conducted.
    • Key risk factors identified include epithelial defects, history of herpetic keratouveitis, and topical corticosteroid use.

    Findings:

    • Gram-negative rods, gram-positive organisms, fungi, and Mycobacterium cheloni were identified as causative agents.
    • Topical antibiotic use did not prevent superinfections in a majority of cases.
    • Epithelial defects were present in all 15 patients, highlighting their critical role.
