    Background:

    • Obliterating atherosclerosis (OA) is a significant vascular disease.
    • Early detection of OA is crucial for effective management.
    • Current diagnostic methods may have limitations in detecting early-stage OA.

    Purpose of the Study:

    • To evaluate the efficacy of photoplethysmography in diagnosing obliterating atherosclerosis.
    • To compare photoplethysmographic measurements with peripheral systolic pressure measurements for OA detection.

    Main Methods:

    • Photoplethysmographic measurements were performed on limbs with documented OA.
    • The distance P between ascending and descending pulse wave limbs was measured.
    • The P/L ratio was calculated to standardize measurements.

    Main Results:

    • Photoplethysmography detected 80.8% of OA without occlusion (OAWO) and 100% of OA with complete occlusion (OAWCO) in young healthy individuals.
    • In older healthy individuals, it detected 69.8% of OAWO and 100% of OAWCO.
    • The P/L ratio method identified a higher number of OAWO cases compared to peripheral systolic pressure measurements.

    Conclusions:

    • Photoplethysmography, using the P/L ratio, is a sensitive method for detecting obliterating atherosclerosis.
    • This technique offers improved detection rates for OAWO compared to peripheral systolic pressure.
    • Photoplethysmography shows promise as a non-invasive diagnostic tool for peripheral artery disease.
