    Summary

    Macro-CK, an atypical creatine kinase (CK) variant, can cause false-positive CK-MB results. Electrophoresis and immunoprecipitation showed fewer false positives compared to column chromatography and immunoinhibition methods for macro-CK detection.

    Background:

    • Macro-CK is an atypical creatine kinase (CK) isoenzyme variant found in approximately 1% of patients.
    • Accurate CK isoenzyme determination is crucial for diagnosing cardiac conditions.

    Purpose of the Study:

    • To evaluate the performance of four different methods in detecting macro-CK.
    • To assess the potential for false-positive CK-MB results in the presence of macro-CK.

    Main Methods:

    • Creatine kinase (CK) isoenzyme analysis was performed on specimens from two patients with macro-CK.
    • Methods included electrophoresis (Corning ACI), column chromatography (Dupont ACA), immunoinhibition (ESKACHEM), and immunoprecipitation (Isomune-CK).

    Main Results:

    • Electrophoresis showed no false-positive CK-MB.
    • Immunoprecipitation yielded 1 false-positive CK-MB in 8 specimens.
