    Summary

    Hypothermia causes hypokalemia by shifting potassium into cells. During controlled hypothermia, only replace measured potassium losses to prevent dangerous hyperkalemia upon rewarming.

    Background:

    • Hypothermia is a medical condition characterized by abnormally low body temperature.
    • Patients with head injuries or brain hypoxia are susceptible to hypothermia.
    • Hypokalemia, a condition of low serum potassium, can occur during hypothermia.

    Purpose of the Study:

    • To investigate the effects of hypothermia on serum potassium levels.
    • To determine the appropriate potassium replacement strategy during therapeutic hypothermia.

    Main Methods:

    • Observational study of six hypothermic patients (30-32°C) with neurological conditions.
    • Two groups: Group 1 received potassium to maintain serum levels >3.5 mEq/l; Group 2 received potassium to replace measured losses.
    • Serum potassium levels and cardiac rhythms were monitored during hypothermia and rewarming.

    Main Results:

    • Hypokalemia (2.3 ± 0.4 mEq/l) was observed in all patients.
    • Group 1 developed hyperkalemia (7.1 ± 0.5 mEq/l) and cardiac arrhythmias upon rewarming.
    • Group 2 did not experience hyperkalemia; minor arrhythmias occurred during hypothermia.
    • Hypothermia causes potassium to shift intracellularly or extravascularly.

    Conclusions:

    • Hypothermia induces hypokalemia through potassium redistribution.
    • Aggressive potassium replacement during hypothermia can lead to dangerous hyperkalemia on rewarming.
    • Potassium therapy during controlled hypothermia (30-32°C) should be limited to replacing measured losses.
