    Summary

This study reports a rare case of multilocular encapsulated intracerebral hematoma with multiple capsules of varying ages. The number of convulsive seizures correlated with the number of hematoma capsules, suggesting a unique clinical presentation.

    Area of Science:

    • Neurology
    • Pathology

    Background:

    • Encapsulated intracerebral hematoma is an exceedingly rare condition, with only two prior cases documented.
    • This report details a unique instance of a multilocular encapsulated intracerebral hematoma.

    Observation:

    • The hematoma featured 9 to 10 distinct capsules of varying sizes.
    • Capsules were easily dissected, grayish-white, tough, and 1-3 mm thick.
    • Contents varied from old, hard hematomas to recent liquid or xanthochromic fluid.

    Findings:

    • The varied contents indicated hematomas developing over different chronic periods.
    • A notable correlation was observed between the frequency of convulsive seizures and the number of hematoma capsules.
    • Potential etiologies include occult vascular malformations or bleeding from capsular sinusoidal layers.

    Implications:

    • This case expands the understanding of rare intracerebral hematoma morphologies.
    • The seizure-hematoma capsule correlation warrants further investigation.
    • Further research into the etiology of multilocular encapsulated intracerebral hematoma is suggested.
