    Background:

    • Orthostatic hypotension (OH) is a debilitating condition often caused by autonomic failure.
    • Current treatments for OH have limited efficacy and significant side effects.

    Observation:

    • Two patients with severe OH due to autonomic failure were implanted with programmable atrial inhibited pacemakers.
    • Pacemakers were programmed for daytime pacing at 95 beats/min and nighttime pacing at 55 beats/min.

    Findings:

    • A two-year follow-up showed a near-complete resolution of orthostatic symptoms.
    • Erect systolic blood pressure increased from 47 mmHg to 85 mmHg post-implantation.
    • Cardiac output increased from 3.0 L/min to 3.8 L/min with pacing.

    Implications:

    • Programmable pacemakers offer a promising therapeutic option for autonomic failure-associated OH.
    • Dynamic pacing strategies can optimize hemodynamic parameters and symptom control.
    • This approach avoids nocturnal tachycardia and hypertension, enhancing patient safety.
