    Evidence-based treatment for first-episode schizophrenia?

    Background:

    • Previous open-trial reports suggested potential clinical improvement in schizophrenic patients undergoing hemodialysis.
    • These reports prompted further investigation into hemodialysis as a treatment for schizophrenia.

    Purpose of the Study:

    • To determine the efficacy of hemodialysis in treating schizophrenia under double-blind, controlled conditions.
    • To rigorously evaluate the therapeutic effects of hemodialysis on schizophrenic patients.

    Main Methods:

    • A double-blind, randomized controlled trial was conducted.
    • Fifteen schizophrenic outpatients were assigned to either a real-sham or sham-real dialysis sequence.
    • Symptom and behavioral data were collected at baseline, crossover, and end of treatment.

    Main Results:

    • Analysis of symptom and behavioral data revealed no significant differences between real and sham dialysis.
    • Repeated measurements and other statistical analyses showed no differential effects of the treatments.
    • The results indicated a lack of therapeutic efficacy for hemodialysis in this patient group.

    Conclusions:

    • Hemodialysis demonstrated no therapeutic efficacy in treating schizophrenia.
    • The study provides experimental evidence contradicting earlier open-trial reports.
    • Hemodialysis is not a recommended treatment for the psychiatric condition of schizophrenia.
