Tracheostomal recurrences after laryngectomy.

    Summary

Preoperative tracheotomy increases the risk of tracheostomal recurrence after laryngectomy. This surgical complication may share a similar cause with recurrence following conservative surgery for laryngeal cancer.

    Area of Science:

    • Otolaryngology
    • Surgical Oncology
    • Pathology

    Background:

    • Laryngectomy is a surgical procedure for laryngeal cancer.
    • Tracheostomal recurrence, a complication post-laryngectomy, requires further investigation.
    • The role of preoperative tracheotomy in recurrence is debated.

    Purpose of the Study:

    • To investigate the etiopathogenesis of post-laryngectomy tracheostomal recurrence.
    • To analyze the specific role of preoperative tracheotomy in tracheostomal recurrence.
    • To compare recurrence patterns between preoperative tracheotomy and inadequate conservative surgery.

    Main Methods:

    • Clinicopathologic study of 681 laryngectomies.
    • Analysis of tumor specimens from patients with tracheostomal recurrence.
    • Review of case histories for patients undergoing conservative surgery followed by laryngectomy.

    Main Results:

    • Tracheostomal recurrence was more frequent in patients with preoperative tracheotomy.
    • Morphological analysis revealed submucosal infiltration in some tumor specimens from preoperatively tracheotomized patients.
    • A high rate of tracheostomal recurrence was observed in patients with endolaryngeal recurrence after conservative surgery.

    Conclusions:

    • Preoperative tracheotomy is associated with a higher incidence of tracheostomal recurrence.
    • The pathogenetic mechanism of tracheostomal recurrence may be similar for both preoperative tracheotomy and inadequate conservative surgery.
    • Further research into surgical techniques and patient selection is warranted.
