    Congenital syphilis in infants can cause unusual liver scan results with radioactivity in the lungs and bones. These findings resolved after antibiotic treatment, highlighting a new aspect of this disease.

    • These unique scintigraphic features are previously undescribed in congenital syphilis.
    • The observed pattern demonstrated diminished hepatic function with compensatory extrahepatic radiocolloid distribution.
    • Complete resolution of these abnormal findings occurred after penicillin therapy.

    Implications:

    • This case expands the spectrum of imaging manifestations for congenital syphilis.
    • Liver scintigraphy can reveal unusual patterns in pediatric syphilitic hepatitis.
    • Prompt diagnosis and treatment with penicillin are crucial for reversing these effects.
