    Summary

    Foreign body arthritis can occur after knee replacement surgery. Treatment with steroids and Yttrium synoviorthesis effectively managed these cases without antibiotics.

    Background:

    • Total knee arthroplasty (TKA) is a common procedure for knee osteoarthritis.
    • Prosthetic joint infections are a significant complication following TKA.
    • Non-infectious inflammatory reactions can also occur after TKA.

    Observation:

    • Three patients presented with acute knee arthritis 3 months to 5 years post-TKA.
    • Infectious etiologies were definitively ruled out in all cases.
    • Cytological and histological analyses indicated foreign body reactions.

    Findings:

    • The arthritis was attributed to polyethylene debris and metal particles from the prosthesis.
    • Patients responded well to local corticosteroid injections and Yttrium-90 synoviorthesis.
    • Antibiotic therapy was not required for successful treatment.

    Implications:

    • Foreign body reactions should be considered in the differential diagnosis of knee arthritis after TKA.
    • Yttrium synoviorthesis may be a valuable treatment option for managing prosthetic debris-induced arthritis.
    • Addressing foreign body granulomas could potentially prevent prosthesis loosening.
