    Summary

    This study reviewed 62 adrenal cortical carcinoma patients, finding that early-stage disease and aggressive surgery significantly improve survival. Adjuvant therapy did not impact outcomes in this historical cohort.

    Background:

    • Adrenal cortical carcinoma (ACC) is a rare and aggressive endocrine malignancy.
    • Understanding prognostic factors and optimal treatment strategies for ACC is crucial.

    Purpose of the Study:

    • To evaluate the prognostic value of operative staging in adrenal cortical carcinoma.
    • To assess the impact of surgical treatment and adjuvant therapy on survival rates.

    Main Methods:

    • Retrospective review of 62 patients with pathologically confirmed adrenal cortical carcinoma operated on between 1960 and 1980.
    • Analysis of patient demographics, tumor characteristics, surgical procedures (curative resection, palliative resection, open biopsy), and adjuvant therapy.
    • Evaluation of operative mortality and 5-year survival rates.

    Main Results:

    • Overall operative mortality was 9.7% and the 5-year survival rate was 16%.
    • Patients with stages I-III lesions and those undergoing curative resections had significantly longer survival than stage IV disease or palliative resections.
    • No significant difference in survival was observed between operations alone versus operations plus adjuvant therapy.

    Conclusions:

    • Operative staging is a valuable prognostic indicator for adrenal cortical carcinoma.
    • Aggressive surgical treatment is recommended for stages I-III lesions.
    • A prospective randomized study on adjuvant treatment for all stages of ACC is warranted.
