    Summary

    A patient developed a sudden cataract after radial keratotomy surgery. The cause of this unexpected complication remains unknown, despite a thorough examination.

    Background:

    • Radial keratotomy is a refractive surgical procedure to correct myopia.
    • The patient, a 31-year-old male, underwent bilateral radial keratotomy.

    Observation:

    • The surgery was uneventful except for a minor corneal microperforation in the left eye.
    • Excellent uncorrected visual acuity was noted postoperatively in both eyes.

    Findings:

    • A sudden cataract developed in the left eye 3.5 months after radial keratotomy.
    • No pre-existing ocular or systemic conditions were identified as causes.
    • Examination revealed no signs of iris or anterior capsular damage.

    Implications:

    • The unexplained cataract development highlights a potential, albeit rare, complication of radial keratotomy.
    • Further research may be needed to understand the mechanisms linking radial keratotomy to late-onset cataracts.
