    Background:

    • Bupivacaine hydrochloride is a common anesthetic agent used for brachial plexus blocks.
    • Prolonged blocks can occur, but the underlying mechanisms require further investigation.

    Observation:

    • A 73-year-old patient experienced a prolonged brachial plexus block after anesthesia induction with a diluted 0.50% bupivacaine hydrochloride solution.
    • Recovery from motor block began 24 hours post-anesthesia, with full recovery observed 30 hours later.

    Findings:

    • The diluted bupivacaine solution had significantly lower osmolality (141 mosm X kg-1) compared to standard commercial solutions.
    • Postoperative examination revealed bilateral slowing of nerve conduction in the brachial plexus, suggesting potential nerve involvement.

    Implications:

    • The low osmolality of the diluted anesthetic may have contributed to the prolonged block and observed nerve conduction changes.
    • Further research is needed to determine if low osmolality causes nerve lesions and to assess the clinical significance of these findings.
