    Background:

    • Left hemiplegia and left-sided neglect commonly occur after stroke, leading to impaired upper extremity function and increased risk of shoulder complications.
    • Dependent positioning of a flaccid upper extremity can cause mechanical stress on the shoulder joint, resulting in pain and limited range of motion (ROM).

    Observation:

    • A simple, inexpensive, battery-powered device was developed to alert patients when their left upper extremity was in a dependent position.
    • The device prompted the patient to reposition the affected limb onto a lapboard, reducing mechanical stress.

    Findings:

    • The patient reported decreased left shoulder pain and increased shoulder ROM after using the device.
    • While not solely attributable to the device due to concurrent therapy, it provided the patient with greater autonomy in managing their condition.

    • This device may enhance awareness of the affected upper extremity in patients with hemiplegia and neglect.
    • Increased limb awareness and improved positioning can potentially lead to better carry-over into daily living activities and reduced secondary complications.
