    Summary

    A rare melanoma case developed in a cellular blue nevus on the ear lobe. Metastases appeared 25 years later, with survival following surgical intervention.

    Background:

    • Cellular blue nevi are benign melanocytic proliferations.
    • Melanoma arising in cellular blue nevi is exceptionally rare.

    Observation:

    • A case of melanoma developing in a cellular blue nevus of the ear lobe is presented.
    • The primary tumor had a comparatively favorable course initially.

    Findings:

    • Metastases to the right parotid salivary gland and lymph nodes occurred 25 years post-excision.
    • The patient survived for 7 years after parotidectomy and cervical adipose tissue dissection.

    Implications:

    • This case highlights the potential for late metastasis from melanoma originating in cellular blue nevi.
    • Long-term surveillance may be warranted for such rare presentations.
