    Background:

    • Investigating the precise locations of cerebellar damage associated with dysarthria is crucial for understanding nondegenerative cerebellar disease.
    • Previous research has indicated a link between cerebellar lesions and speech impairments, but specific areas require further elucidation.

    Observation:

    • A review of 162 patients with focal cerebellar lesions, including surgical and autopsy findings, was conducted.
    • Speech function was assessed in 122 patients, with 31 exhibiting dysarthria.

    Findings:

    • Dysarthria was most commonly associated with exclusively or predominantly left cerebellar hemisphere disease (22 patients).
    • Damage to the superior portion of the left cerebellar hemisphere showed the strongest correlation with affected cerebellar speech function.
    • No significant correlation was found between vermal damage extent and abnormal speech development.

    Implications:

    • Identifying specific cerebellar regions linked to dysarthria can improve diagnostic accuracy and targeted treatment strategies.
    • This research contributes to a deeper understanding of the neuroanatomy underlying speech production and its disruption in cerebellar disorders.
