Verapamil and digoxin: interactions in the rat.

J Kaplanski, E Weinhouse, M Topaz

Research Communications in Chemical Pathology and Pharmacology

    Summary

Verapamil can help manage digoxin-induced arrhythmias and reduce mortality. However, verapamil pretreatment significantly increases digoxin concentrations in plasma and tissues, potentially enhancing toxicity.

    Area of Science:

    • Pharmacology
    • Cardiovascular Research

    Background:

    • Verapamil and digoxin are frequently co-administered.
    • Clinical observations suggest verapamil may potentiate digoxin toxicity.

    Purpose of the Study:

    • To investigate verapamil's effects on digoxin-induced tachyarrhythmias.
    • To assess verapamil's influence on digoxin pharmacokinetics in rats.

    Main Methods:

    • Rats received digoxin, followed by verapamil to observe antiarrhythmic effects.
    • Separate cohort: rats pretreated with verapamil, then administered digoxin.
    • Digoxin levels measured via radioimmunoassay in plasma and various tissues.

    Main Results:

    • Verapamil converted 75% of digoxin-induced tachyarrhythmias to sinus rhythm.
    • Verapamil significantly reduced digoxin-induced mortality.
    • Verapamil pretreatment doubled digoxin concentrations in plasma, heart, liver, and muscle.

    Conclusions:

    • Verapamil exhibits both antiarrhythmic effects against digoxin toxicity and pharmacokinetic interactions.
    • Pretreatment with verapamil increases systemic and tissue digoxin levels, warranting careful consideration in combined therapy.
