    Area of Science:

    • Pulmonology
    • Radiology
    • Pathology

    Background:

    • Sarcoidosis is an inflammatory disease that can affect multiple organs, commonly the lungs.
    • Acute sarcoidosis typically presents with hilar adenopathy and normal bronchoalveolar lavage lymphocyte counts.
    • Lung cavitation is an uncommon manifestation of acute sarcoidosis.

    Observation:

    • A 25-year-old woman presented with hilar adenopathy and bilateral cavitary lung lesions.
    • Radiographic abnormalities were attributed to sarcoidosis.
    • Bronchoalveolar lavage revealed normal lymphocyte counts and increased multinucleated giant cells.

    Findings:

    • The patient's presentation deviated from typical acute sarcoidosis.
    • The presence of lung cavitation with specific bronchoalveolar lavage findings suggests an atypical form of sarcoidosis.
    • This case highlights a rare manifestation of sarcoidosis.

    Implications:

    • Clinicians should consider atypical sarcoidosis in the differential diagnosis of cavitary lung disease.
    • Further research may be needed to understand the specific characteristics and management of this atypical sarcoidosis variant.
    • This case expands the understanding of sarcoidosis presentations and diagnostic challenges.
