    Summary

    In some myopathy patients, the heart works harder than normal during exercise due to an abnormal increase in cardiac output (Q) relative to oxygen uptake (VO2). This suggests potential circulatory regulation issues in skeletal muscle caused by certain myopathies.

    Background:

    • Normally, increased cardiac output (Q) during exercise is tightly coupled to oxygen uptake (VO2).
    • The typical ratio of increase in Q to VO2 (delta Q/delta VO2) is approximately 5-6 L/L.

    Purpose of the Study:

    • To investigate the relationship between cardiac output and oxygen uptake during exercise in patients with myalgia and myopathies.
    • To identify if certain myopathies are associated with abnormal circulatory responses during physical exertion.

    Main Methods:

    • Cardiac output and oxygen uptake were measured at rest and during bicycle exercise.
    • Patients included 7 with myalgia and 15 with various myopathies.

    • Resting circulation was normal in all participants.
    • Most myalgia and myopathy patients exhibited a normal delta Q/delta VO2 ratio during exercise.
    • Four patients (dermatomyositis, phosphorylase deficiency, carnitine deficiency, ocular myopathy) showed an excessive increase in cardiac output relative to oxygen uptake (high delta Q/delta VO2).

    Conclusions:

    • A subset of myopathy patients displays a hyperkinetic circulatory response during exercise.
    • This abnormal response, characterized by a high delta Q/delta VO2, may indicate aberrant circulatory regulation by skeletal muscle in specific myopathies.
