Headache as an epileptic manifestation.

    Summary

    Headaches can be a symptom of seizure activity originating in the brain's limbic system. Surgical removal of the seizure focus in the right temporal lobe successfully resolved these headache episodes.

    Background:

    • Headaches are a common neurological symptom with diverse etiologies.
    • Epilepsy is characterized by recurrent seizures, but headache as an ictal manifestation is less commonly recognized.

    Observation:

    • Two patients presented with headaches that were correlated with electroencephalographic (EEG) ictal activity.
    • Depth electrode recordings confirmed the epileptic nature of the headaches.

    Findings:

    • The epileptogenic focus was localized to the right limbic system in both cases.
    • Right temporal lobectomy resulted in complete resolution of the headaches in both patients.

    Implications:

    • This study highlights the importance of considering epilepsy when evaluating patients with intractable headaches.
    • Identifying and treating the epileptogenic focus can effectively manage headache symptoms associated with ictal activity.
