    Summary

    Fetal alcohol myopathy is a unique muscle disorder identified in infants exposed to alcohol in utero. This condition causes muscle weakness and structural abnormalities at the cellular level.

    Background:

    • Investigated a unique myopathic process in neonates born to mothers with alcohol use disorder.
    • Observed depressed in utero activity in all affected infants.

    Observation:

    • Muscle biopsies revealed hypotrophy, type II fiber dominance, and central nuclei.
    • Ultrastructural examination showed marked sarcomeric dysplasia.

    Findings:

    • Sarcomeres exhibited variable length and diameter.
    • Persistent Z-band material and actin were found in debris.
    • A decrease in myosin filaments was noted.
    • Myocyte involvement ranged from focal to holocellular dysplasia.

    Implications:

    • The observed ultrastructural changes represent a unique myopathy.
    • This constellation of findings supports the designation of "fetal alcohol myopathy."
