    Summary

    Severe colitis caused by Clostridium difficile toxins can lead to extreme mural edema, a finding most typical of this condition. This observation suggests investigating C. difficile in acute colitis cases with significant edema.

    Background:

    • Clostridium difficile infection (CDI) is a significant cause of antibiotic-associated diarrhea and colitis.
    • Severe cases of pseudomembranous colitis may necessitate surgical intervention.

    Observation:

    • Analysis of colonic specimens from three patients with severe pseudomembranous colitis revealed marked, diffuse mural edema extending into the muscularis propria.
    • This extreme edema was present in areas both with and without pseudomembranes.

    Findings:

    • Massive and diffuse mural edema appears to be a characteristic pathological finding in Clostridium difficile-associated colitis.
    • The edema may result from a combination of toxin-induced vascular effects and aggressive fluid resuscitation.

    Implications:

    • The presence of extensive mural edema in acute colitis should prompt investigation for C. difficile toxins and recent antibiotic use.
    • Recognizing this specific pathological feature can aid in diagnosing and managing severe CDI.
