    Summary

    Bile duct papillomatosis, a rare condition, can occur within the liver and may show atypical cell changes. Radical surgery, like hepatic lobectomy, offers a potential cure for this challenging biliary tract disease.

    Background:

    • Intrahepatic bile duct papillomatosis is a rare neoplastic proliferation of the biliary epithelium.
    • It can present with or without nuclear atypia, posing diagnostic challenges.

    Observation:

    • A case of intrahepatic bile duct papillomatosis with nuclear atypia in a young male is presented.
    • The patient underwent radical surgical excision, including left hepatic lobectomy and bile duct resection.

    Findings:

    • Review of 14 prior cases reveals a high rate of local recurrence and progression to fatal obstructive jaundice and cholangitis.
    • Malignant transformation is uncommon, with only two confirmed cases reported in the literature.

    Implications:

    • Radical surgical resection represents a potentially curative treatment for biliary papillomatosis.
    • Early diagnosis and aggressive surgical management are crucial for improving patient outcomes in this rare condition.
