Lactulose-induced hypernatremia.

A A Nanji, R W Lauener

    Summary

Severe hypernatremia can occur in patients treated with lactulose, a common medication for hepatic encephalopathy. Close electrolyte monitoring and dose reduction are crucial, especially when free water access is limited.

    Area of Science:

    • Medicine
    • Gastroenterology
    • Nephrology

    Background:

    • Hepatic encephalopathy is a serious complication of liver disease.
    • Lactulose is a standard treatment for hepatic encephalopathy, promoting ammonia excretion.

    Observation:

    • A case of severe hypernatremia was observed in a patient receiving lactulose therapy.
    • This adverse effect was linked to the patient's lactulose dosage and limited free water intake.

    Findings:

    • Lactulose can induce hypernatremia through osmotic effects and metabolic byproducts.
    • The risk is elevated in patients receiving high doses without adequate hydration.

    Implications:

    • Clinicians should be vigilant for hypernatremia in patients on lactulose.
    • Electrolyte monitoring and prompt lactulose dose adjustment are recommended to prevent severe complications.
