Spurious pseudomembranous enteritis in strongyloidiasis.

J M Morse, A R Laurain, E Thomas

    The American Journal of Gastroenterology
    |February 1, 1984

    Summary

A fatal case of Strongyloidiasis was diagnosed postmortem. Autopsy revealed a unique "membrane" in the small bowel composed of barium sulfate and Strongyloides stercoralis nematodes.

    Area of Science:

    • Medical Parasitology
    • Gastroenterology
    • Pathology

    Background:

    • Strongyloidiasis is a parasitic infection caused by the nematode Strongyloides stercoralis.
    • The infection can remain asymptomatic for decades or present with varied gastrointestinal symptoms.
    • Bowel hypomotility is a recognized complication of chronic Strongyloidiasis.

    Observation:

    • A fatal case of Strongyloidiasis was initially unsuspected until postmortem examination.
    • Autopsy revealed gross changes in the small bowel mimicking pseudomembranous enteritis.
    • A unique "membrane" was identified, composed of inspissated barium sulfate, nematodes, and proteinaceous material.

    Findings:

    • The "membrane" was formed by barium sulfate from a prior gastrointestinal series and Strongyloides stercoralis.
    • This finding is considered unique in the context of Strongyloidiasis pathology.
    • The inspissated barium sulfate likely contributed to bowel stasis and exacerbated the parasitic infection.

    Implications:

    • This case highlights a potential diagnostic challenge in Strongyloidiasis.
    • Bowel hypomotility in Strongyloidiasis may lead to unusual complications involving ingested substances.
    • Understanding this interaction could inform diagnostic and management strategies for severe Strongyloidiasis.
