    Summary

    Patients with vitiligo showed reduced skin sensitivity to dinitrochlorobenzene (DNCB) in affected patches compared to normal skin. This suggests potential functional changes in skin cells within vitiligo lesions.

    Background:

    • Vitiligo is a condition characterized by depigmentation of the skin.
    • The immune system's role in vitiligo pathogenesis is complex and not fully understood.
    • Langerhans' cells are critical immune cells in the skin involved in antigen presentation.

    Purpose of the Study:

    • To investigate contact hypersensitivity reactions in patients with vitiligo.
    • To compare immune responses in vitiliginous skin versus normal skin.
    • To explore potential mechanisms underlying altered immune function in vitiligo.

    Main Methods:

    • Thirty patients with vitiligo (segmental and generalized types) were sensitized with dinitrochlorobenzene (DNCB).
    • DNCB challenge tests were conducted on both vitiliginous patches and normal skin sites.
    • Tuberculin reactivity was also assessed in vitiliginous lesions.

    Main Results:

    • Diminished contact sensitivity reactions to DNCB were observed in vitiliginous patches for both patient groups.
    • Normal delayed hypersensitivity responses to DNCB were noted in normal skin sites of the same patients.
    • Tuberculin reactivity was not suppressed in vitiliginous lesions, indicating localized immune alterations.

    Conclusions:

    • Vitiliginous skin exhibits diminished contact reactivity to DNCB.
    • This reduced reactivity may stem from functional alterations in skin's Langerhans' cells.
    • Alternatively, changes in skin carrier proteins within lesions could contribute to the observed immune modulation.
