    Summary

    Bromide intoxication, a rare condition, was diagnosed in a critically ill patient with neurological issues. A gastric bezoar containing pill fragments was identified as the cause of prolonged bromide elevation.

    Background:

    • Bromide intoxication is an uncommon condition.
    • It can present with nonspecific neurologic abnormalities and elevated serum chloride levels.

    Observation:

    • A critically ill patient presented with neurologic abnormalities and elevated chloride levels.
    • Serum bromide levels were also significantly elevated and persisted for over 12 days.

    Findings:

    • Endoscopy revealed a gastric bezoar composed of identifiable pill fragments.
    • Gastric washings from the bezoar fragments showed high bromide concentrations.

    Implications:

    • This case highlights a unique cause of prolonged bromide intoxication.
    • Gastric bezoars should be considered in the differential diagnosis of unexplained hyperchloremia and intoxication.
    • Early identification and removal of the bezoar are crucial for patient recovery.
