    Background:

    • Chronic lymphocytic leukemia (CLL) is a common B-cell malignancy.
    • Immune dysregulation, including hypogammaglobulinemia, is frequently observed in CLL patients.
    • The typical presentation and progression of CLL vary significantly among individuals.

    Observation:

    • A 66-year-old female presented with recurrent respiratory infections, severe hypogammaglobulinemia, and impaired cell-mediated immunity.
    • Abdominal lymphadenopathy and splenomegaly were noted, prompting investigation for lymphoid malignancy.
    • Initial explorative laparotomy did not reveal evidence of malignancy.

    Findings:

    • A diagnosis of chronic lymphocytic leukemia (CLL) was established after a two-year follow-up period.
    • The patient exhibited significant hypogammaglobulinemia two years prior to the CLL diagnosis.
    • This temporal association of hypogammaglobulinemia preceding CLL development is unprecedented in medical literature.

    Implications:

    • This case suggests that profound immune deficiency may precede overt clinical diagnosis of CLL.
    • Understanding this early immunophenotype could lead to earlier detection strategies for CLL.
    • Further research is warranted to explore the pathogenetic link between hypogammaglobulinemia and CLL onset.
