    Area of Science:

    • Oncology
    • Pulmonology
    • Hematology

    Background:

    • Multiple myeloma is a plasma cell malignancy.
    • Pleural effusions are uncommon manifestations of multiple myeloma.

    Observation:

    • A patient presented with a left pleural effusion.
    • Initial imaging revealed an osteolytic lesion on the left clavicle.

    Findings:

    • Pleural fluid cytology confirmed malignant plasma cells.
    • Pleural biopsy demonstrated direct pleural involvement by IgA multiple myeloma.
    • The pleural effusion was directly caused by tumor infiltration.

    Implications:

    • This case highlights a rare pleuritic presentation of IgA multiple myeloma.
    • Emphasizes the importance of considering myeloma in the differential diagnosis of unexplained pleural effusions.
    • Suggests direct pleural involvement can occur in multiple myeloma.
