    Background:

    • A case of a patient with a relatively asymptomatic, insidiously progressive, massive hepatic enlargement is presented.
    • The clinical presentation mimicked other causes of liver masses.

    • Ultrasonography and computed tomography (CT) were instrumental in characterizing the liver lesion.
    • Distinctive gross and microscopic pathological findings confirmed the diagnosis of biliary cystadenoma.

    Implications:

    • This case highlights the importance of considering rare diagnoses like biliary cystadenoma in patients with massive liver enlargement.
    • Accurate imaging and pathological examination are essential for correct diagnosis and management of these rare liver tumors.
