    Summary

    Area of Science:

    • Gastroenterology
    • Surgical Gastroenterology
    • Clinical Medicine

    Background:

    • Peptic ulcer disease is a common condition.
    • Bleeding peptic ulcers represent a significant cause of morbidity and mortality.
    • Optimal treatment strategies for bleeding peptic ulcers remain an area of clinical interest.

    Purpose of the Study:

    • To analyze treatment outcomes for bleeding peptic ulcer patients.
    • To compare mortality and rebleeding rates between surgical and medical management.
    • To identify risk factors associated with adverse outcomes in bleeding peptic ulcer disease.

    Main Methods:

    • Retrospective analysis of 430 patients treated for bleeding peptic ulcer between 1972 and 1981.
    • Categorization of patients into surgical (n=210) and medical (n=220) treatment groups.
    • Evaluation of mortality and rebleeding incidence based on surgical procedure (gastrectomy, vagotomy) and patient demographics (age, ulcer location).

    Main Results:

    • Surgical group: High mortality observed in posterior duodenal ulcers and post-gastrectomy compared to vagotomy. Emergency operations also showed high mortality.
    • Rebleeding rates were comparable between gastrectomy and vagotomy.
    • Medical group: Elderly patients experienced significantly higher mortality.

    Conclusions:

    • Posterior duodenal ulcers and gastrectomy are associated with increased mortality in surgical management of bleeding peptic ulcers.
    • Emergency surgery for bleeding peptic ulcers carries a high mortality risk.
    • Age is a critical factor influencing mortality in medically managed bleeding peptic ulcer patients.
