    Summary

    Blunt trauma frequently causes diaphragm rupture, with most cases involving left-sided injuries and herniation of abdominal organs into the chest. Right-sided ruptures correlate with higher morbidity and mortality due to associated injuries.

    Background:

    • Diaphragmatic rupture is a severe consequence of blunt force trauma.
    • Prompt diagnosis and surgical intervention are critical for patient outcomes.

    Observation:

    • This study analyzed 31 patients with blunt diaphragmatic rupture.
    • Left-sided ruptures (23) were more common than right-sided (7).
    • Viscus herniation into the chest occurred in 30 patients.

    Findings:

    • Right-sided ruptures were linked to increased morbidity and mortality, often due to associated injuries.
    • Chest X-rays were abnormal in all cases; bowel was visible in the chest in 11 patients.
    • Diagnostic imaging like barium studies and scans aided diagnosis in select cases.

    Implications:

    • Preoperative diagnosis was achieved in 25 patients, but one-third of diagnoses were delayed.
    • Thorough diaphragmatic inspection during exploratory laparotomy is crucial for detecting ruptures.
    • Early detection prevents chronic herniations and improves patient prognosis.
