    Summary

    Resistant tennis elbow surgery outcomes were similar whether lengthening the extensor carpi radialis brevis tendon or decompressing the radial tunnel. Anatomical study suggests relieving supinator tension is key, not nerve decompression.

    • To compare surgical outcomes for resistant tennis elbow.

    • Retrospective study of 50 resistant tennis elbow cases.
    • 37 treated with extensor carpi radialis brevis tendon lengthening.
    • 13 treated with radial tunnel decompression.

    • Similar positive surgical outcomes in both treatment groups.
    • Anatomical findings indicate relief from dividing the supinator's fibrous arch.
