    Summary

Ruptured hydatid cysts can cause acute cholangitis (gallbladder inflammation). Prompt diagnosis via ultrasonography and surgical intervention, including echinococcal cyst removal, leads to successful outcomes with no mortality in these cases.

    Area of Science:

    • Hepatobiliary Surgery
    • Parasitology
    • Gastroenterology

    Background:

    • Acute cholangitis is a serious biliary infection.
    • Hydatid cysts, caused by Echinococcus granulosus, can rupture into the biliary tract.
    • Ruptured hydatid cysts present a diagnostic and therapeutic challenge in hepatobiliary surgery.

    Purpose of the Study:

    • To describe the clinical presentation, diagnosis, and surgical management of acute cholangitis secondary to ruptured hydatid cysts.
    • To evaluate the outcomes of a specific surgical approach for this condition.

    Main Methods:

    • Retrospective case series of four patients diagnosed with acute cholangitis due to ruptured hydatid cysts.
    • Preoperative diagnosis primarily utilized ultrasonography.
    • Surgical management involved cholecystectomy, choledochotomy, choledochoduodenostomy, and echinococcal cyst removal.

    Main Results:

    • Three out of four patients were diagnosed preoperatively using ultrasonography.
    • All four patients underwent surgical treatment, with no operative mortality.
    • One patient required a second operation due to incomplete echinococcal cyst removal during the initial surgery.

    Conclusions:

    • Acute cholangitis resulting from ruptured hydatid cysts requires prompt diagnosis and surgical intervention.
    • A comprehensive surgical approach including cyst removal is effective in managing this condition.
    • Ultrasonography is a valuable tool for preoperative diagnosis of ruptured hydatid cysts causing cholangitis.
